An Arm & a Leg welcomes provisional reservations but can only confirm a booking in writing when the following have been received:

From NHS and other publicly funded patients:

• GP or Consultant referral
• Completed Booking Form
• Patient’s signed Patient Questionnaire (on form supplied by us)
• Written pre-authorisation of treatment and undertaking to pay the full cost of the Treatment Programme and Supplies (bandages etc and compression garments) from the relevant Primary Care Trust, Local Health Board or Health Executive
• GP compiled medical history and confirmation of medication

From self-funded and privately insured patients

• GP or Consultant referral
• Completed Booking Form
• Patient’s signed Patient Questionnaire (on form supplied by us)
• GP compiled medical history and confirmation of medication
• Non-refundable deposit of £3,000

50% of the cost of the appropriate Treatment Programme is due 6 weeks before the commencement of the clinic. If this deadline is not met, the booking will be cancelled and the deposit lost. The balance of the cost of the Treatment programme together with a deposit against the cost of bandages and compression garments is payable on the first day of clinic.

Insured patients may provide written pre-authorisation and undertaking to pay for the Treatment Programme and Supplies from their insurance company in lieu of payment but they remain liable to An Arm & a Leg and are responsible for ensuring timely settlement by their insurance company.
